Health Care has shifted. As a result, quality of care has declined. The focus is on acute care. Continuity of Care is needed now more than ever. We believe it can help provide better quality & outcomes of of care. Costs for individuals and society can also be reduced.
Care is no longer the focus. The focus is on payment. Sadly, this does not translate into better care. Good outcomes come through communication & coordination with doctors. Doctors should maintain relationships with patients and families.
